With more recent improvements in the survival of extremely preterm infants in the 22-25 weeks range, the optimal approaches to initial respiratory support will be reviewed to minimize gaps that may exist in current knowledge or existing practice.

1. Describe the basic aspects of lung development at 22-25 weeks and the impact of potentially toxic insults in the ex-utero environment.
2. Discuss the unique operative features of high-frequency and conventional ventilators and how they optimize early lung inflation.
3. Identify deficiencies in evidence from past trials regarding optimal approaches to early ventilator care in the most immature infants.
